Surgical management of chronic suppurative otitis media: A 3-year experience

Abstract: Background / Objective : To determine the outcome of surgery for chronic suppurative otitis media at our department over 3 years, between 2001 and 2003. The outcome measures were; achievement of dry ear, closure of tympanic membrane perforation and improvement in hearing for patients who had myringoplasty. Methods :A retrospective review of case notes of patients with chronic suppurative otitis media (cholesteatoma and non-cholesteatomatous disease) operated upon by 2 consultant ENT surgeons in our department over a 3 year period. The operations performed were mastoidectomy and type I tympanoplasty or myringoplasty. The results were analysed and presented. Results :Seventy three patients were operated upon for cholesteatoma and non-cholesteatomatous chronic otitis media during this period, there were 41 males and 32 females with a sex ratio of 1:1.3. Age range was 9 to 84 years with the mean age of 37 years. 17 of them were children aged 9 to 15 years (24%). 25 patients (34.2%) had cholesteatoma and 48 (65.8%) had non-cholesteatomatous disease 41 (56.2%) had myringoplasty and 29 (39.7%) had mastoidectomy, and 3 (4.1%) patients had tympanomastoidectomy. 62.5% (n=20) mastoid cavities were dry at 12 months follow up. Perforation closure was achieved in 89% (n=34). Binaural hearing was achieved in 6 patients and in 4 patients the operated ear became the better hearing ear; air-bone gap was closed completely in 8 patients (19.5%), and in 36.6% (n=15) the gap was closed to within 10dB. There was no deterioration in hearing in the operated ears. One patient presented with House Brachman grade 2 facial palsy pre-operatively which worsened to grade 4 post operatively but improved to pre-operative grade at 12month follow up. Conclusion : Chronic suppurative otitis media is a disease that is still with us, and at our department, the surgery has a high success rate, which is also comparable in adults and children.

Abstract: Inulin, an oligosaccharide produced by several plants, has been shown to enhance the viability of probiotic cultures in milk through storage. Jerusalem artichoke ( Helianthus tuberosus L.) is an interested prebiotic because its tuber has risen content of inulin and fructo-oligosaccharides. This study was aimed to: 1) set the effect of Jerusalem artichoke in deferent concentrations (2.5% & 5%) on the growth of probiotic Lb. acidophilus P106 in the bio-yoghurt during cold storage at 5 and sensory evaluation of probiotic yoghurts; 2) study the effect of feeding with this synbiotic fermented milk on diabetic mice. It could be concluded that the Jerusalem artichoke influenced the growth of Lb. acidophilus P106 and 5% (w/v) Jerusalem artichoke was given the highest growth and sensory evaluation. On the other hand, no serious adverse effects were observed; the reduction of blood glucose was observed at the termination of empirical phase, also, high level (5%) of Jerusalem artichoke led to more reduction of blood glucose, cholesterol levels and total lipids compared with control.

Abstract:

In this paper, a theoretical treatment of the stability behavior of the zero solution of nonlinear damped oscillator in the vectorial case is investigated. We study the sufficient conditions for the boundedness of solution of the nonlinear damped vectorial oscillator and the conditions for the stability of the zero solution to be uniformly stable as well as asymptotically stable.

Synthesis and Characterization of Nanosized Pigments

Abstract: The work in this paper was devoted to investigating some nanosized iron oxide pigments prepared by microemulsion technique. The role of concentration of iron salt and surfactant (cetyltrimethylammonium bromide) on the produced iron oxide was studied. The techniques employed to characterize the samples were thermogravimetric analysis, X-ray diffractometry, transmission electron microscope, infrared spectroscopy, and diffuse reflectance spectroscopy. The results revealed that the particle size of the prepared sample using 0.2?M iron sulfate and 3.2?wt% of surfactant was in the range 7–9?nm. Increasing the concentration of either iron salt or the surfactant increased the particle size of the obtained ferric oxide. The diffuse reflectance measurements showed that the charge transfer/electron pair transition absorption peak, which is closely related to the reddish color of the oxide, was shifted to a longer wavelength (blue shift) by decreasing the dimension of the particles. The samples were tested as pigments. They showed different tints of red color and were found to be promising for applications as pigments in the field of paint manufacturing. 1. Introduction Nanostructured iron oxides are of a great interest when compared with their bulk counterpart due to their large surface area, high rate of reactivity, and due to the possibility of enhancing environmental friendly reactions. They have superior characters in nontoxicity, chemical stability, durability, and low costs [1]. Due to these properties, they find wide applications in the fields of nanoscience and nanotechnology as pigments, enhancing storage capacity in electronic recording devices, catalysts in some industries, and contrasting agents in magnetic resonance imaging [2–4]. Iron oxide nanoparticles with various structures, sizes, and morphologies have been attained. It is now well known that the morphology and size of the particles have a great impact on their chemical and physical properties. In order to control particle size and obtain monodispersed nanoparticles with a narrow size distribution, various techniques have been employed [5]. Among these techniques, water-in-oil (W/O) microemulsions or reverse micelles are particularly good reaction media for fabricating metal oxide nanoparticles. Anatomic Single Bundle Anterior Cruciate Ligament Reconstruction Using Two Cross Pins: Cadaveric Study and Clinical Application  [PDF]

Abstract: Background: Anatomic single bundle is a widely accepted technique for anterior cruciate ligament (ACL) reconstruction. The research question: are transverse pins safe to fix the ACL graft on the femoral side in anatomic single bundle ACL reconstruction? Material and Methods: Ten cadaveric femoral dry bones were tested in this study. Thirty mm long sockets were prepared in the center of the anatomic ACL footprints. Transverse pin guide for soft tissue graft was used to prepare pin sites through the femoral sockets using three different positions. The positions (A) and (B) had a lateral entry point and differ in their inclination in relation to the joint line; while (A) has a 15° open laterally plane, (B) has a 15° closed laterally. The last position (C) had a medial entry point for the transverse pins. The relations of the transverse pins to the anatomic landmarks (lateral epicondyle and articular cartilage) in addition to the path of the pins within the femoral sockets were recorded. Results: Pins inserted while the guide in position (A) crossed the center of the femoral sockets and were fully seated within the bone of the distal femur. Distal pins inserted through inclination (B) were found to penetrate the posterior cortex of the distal femur proximal to the lateral femoral condyle. Pins inserted from the medial side (C) achieved central position through the femoral tunnel. The entry point for transverse pins through lateral entry points was very close to the attachment of the lateral collateral ligament at the lateral epicondyle while medial entry point avoided important structures. Conclusion: Transverse pins inserted through the lateral entry point carry the risk of penetrating the posterior cortex of the femur and/or damage the attachment of the lateral collateral ligament. Medial entry point may give a safer approach but needs further adjustments of the guides and clinical trials of the technique.

Abstract: Fuzzy Logic Control (FLC) has gained a great demand in process control applications. Fuzzy Logic (FL) technology enables the use of engineering experience and experimental results in designing an expert system capable of handling uncertain or fuzzy quantities. This paper presents a comprehensive review of FLC in the field of Direct Current (DC) motor drive systems. Firstly, the principles of fuzzy logic theory will be briefly presented. Secondly, the employment of the FL techniques in a control system will be outlined. The concept of FLC can be extended for application to different DC motor drives such as: series, separately, shunt and permanent magnet DC motor. The limitations of FLC when applied to DC motor drives have been widely reported in the literature. This article also cites these limitations as well as the advancements in solving them through, for example, the genetic algorithms and the neural networks techniques.
Productivity of Wheat Cultivars as Affected by Seeding Methods and Reflectant Application under Water Stress Condition

Abstract: A field experiment was executed on two wheat varieties (Sakha 93 and Giza 168) grown in clay loam soil under limited water condition at the agricultural experimental station of National Research Centre, Shalakan, Kalubia Governorate, for improving water stress tolerance of wheat plant, using a reflectant (MgCO3), during plant growth. Water shortage had a drastic effect on plant growth, yield and nutrient content, however reflectant treatment under limited water condition and different cultivation methods (ridges, drilling and hills) at milky and maturity stages showed great variations. It was proved that reflectant application increased plant yield to match with normal irrigation condition although growth parameters and nutrient contents did not show a clear significant improvement. Sakha 93 wheat variety surpassed Giza 168 in resisting water stress. The drilling cultivation method surpassed ridges and hills, also a clear increase in K+ content was observed in cultivation under water shortage condition and reflectant application.
Immunohistochemical detection of laminin-1 and Ki-67 in radicular cysts and keratocystic odontogenic tumors

Abstract: Thirteen cases of RCs and twelve cases of KCOTs were included in this study. Antibodies against laminin-1 and Ki-67 were used as primary antibodies.ten cases out of thirteen cases of RCs were immunopositive to laminin-1. The immunonegative cases of RCs showed high degree of inflammation inside the connective tissue wall. One case out of twelve cases of KCOTs was immunopositive to laminin-1 and the rest were immunonegative. Seven cases out of thirteen cases of RCs showed immunopositivity for Ki-67 with increased numbers of immunopositive cells when the inflammation was severe in the connective tissue wall. All KCOTS were immunopositive to Ki-67.The benign nature of radicular cysts and the aggressive behavior of keratocystic odontogenic tumors could be explained by the expression of laminin and Ki-67. Laminin-1 and Ki-67 could be valuable markers for the prediction of the biologic behavior of cystic lesions.Radicular cysts are a direct sequel to chronic apical periodontitis following the death of dental pulp [1]. The epithelial rests of Malassez in periapical granuloma may be stimulated to proliferate by inflammatory stimuli [2]. The morphological aspects of the epithelium have been considered to reflect the functional activity of the RCs [3]. RCs depict a thin, regular and atrophic layer of stratified squamous epithelium, usually with mild to moderate inflammatory reaction [4].
